ฉีดยาชาครั้งแรก
ในการผ่าตัดปลูกผมถาวรเราจะต้องมีการฉีดยาชา 2 ครั้ง โดยครั้งแรกเราจะฉีดยาชาเพื่อให้บริเวณท้ายทอยไม่มีความรู้สึก เพื่อจะได้สามารถเจาะนำกราฟที่แข็งแรงออกมาได้ โดยบริเวณท้ายทอย คุณหมอจะมีเทคนิคการฉีดยาชาต่างกันไป เช่น การฉีดแบบ 4 จุด (nerve block) เป็นการฉีดบริเวณต้นขั้วของเส้นประสาท greater occipital nerve และ lesser occipital nerve ทำให้ส่วนปลายไม่ได้รับความรู้สึกเจ็บ เป็นวิธีที่ดีเนื่องจากไม่จำเป็นต้องใช้ยาชาเยอะ ซึ่งจะทำให้เสี่ยงต่อการได้รับยาชาเกินขนาดและลดอาการไม่พึงประสงค์ได้ อีกทั้งยังเป็นวิธีที่เจ็บน้อยกว่าเพราะไม่ต้องโดนเข็มแทงเยอะด้วย
การฉีดแบบ 10-20 จุด (ring block) เป็นการฉีดยาชาเข้าไปรอบบริเวณที่จะทำการเจาะกราฟออกมา โดยเป็นการฉีดกระจายทั่วๆ ทำให้ผิวหนังชาบริเวณศีรษะชาด้วยฤทธิ์ของยาชาปกติเอง วิธีการนี้จะเจ็บกว่าเล็กน้อยเพราะว่าต้องโดนเข็มแทงค่อนข้างหลายจุด และยังเสี่ยงต่อการได้รับยาเกินขนาดมากกว่า
ฉีดยาชาครั้งที่สอง
การฉีดยาชาครั้งที่สองเป็นการฉีดเพื่อให้ผิวหนังบริเวณด้านหน้าเราต้องการจะปลูกไม่มีความรู้สึก ซึ่งการฉีดยาชาบริเวณด้านหน้าก็สามารถแบ่งเป็น 2 เทคนิคเช่นเดียวกันกับการฉีดด้านหลัง
การฉีด nerve block บริเวณด้านหน้าก็จะเป็นการฉีด 4 จุดเช่นกัน ซึ่งจะเป็นการฉีดยาชาเข้าที่ต้นขั้วของเส้นประสาท supratrochlear nerve และ supraorbital nerve ทำให้คนไข้ชาตั้งแต่บริเวณหัวคิ้ว ขึ้นไปจนถึงกลางศีรษะ เป็นวิธีที่เจ็บน้อยเนื่องจากแทงเพียง 4 จุดและเสี่ยงน้อยกว่าเนื่องจากไม่ต้องใช้ยาเยอะ
การฉีดแบบ ring block บริเวณด้านหน้าก็ต้องมีการฉีด 10-20 จุดเช่นกัน จะเจ็บมากกว่าแบบแรก เสี่ยงต่อการได้รับยาเกินขนาดมากกว่า และการฉีด ring block บริเวณด้านหน้า ยังทำให้มีโอกาสเห็นรอยช้ำบริเวณหน้าผากอีกด้วยครับ

The nerve block injection in the front area is also a 4-point injection, which involves injecting anesthetic into the nerve stem. The supratrochlear nerve and supraorbital nerve cause the patient to be numb from the eyebrow area. up to the middle of the head. It is a method that is less the pain because it only pierces 4 points and is less the risk because it does not require a lot of medicine.
Ring block injections in the front area also require injections of 10-20 points, which will be more painful than the first type. There is a greater risk of overdose and ring block injections in the front area. It also gives a chance to see a bruise on the forehead.
